A 21-year-old woman presents to the colorectal clinic with a change of bowel habit and mucus diarrhoea. At sigmoidoscopy you note mucosal inflammation. Explanatory Notes:
Crohn's disease is characterised by transmural inflammation, with fissures, cobblestoning and skip lesions. Granulomas (collections of macrophages inc epithelioid cells) are almost pathognomonic for Crohn's disease.
Both forms of IBD are associated with mucosal inflammation, bloody or mucus diarrhoea and occur in the young adult.
