A 26-year-old woman at 36 weeks of gestation in her first pregnancy presents because of headache and right-sided upper abdominal pain of two days duration. The pregnancy has been normal until now. Her BP is 145/90mmHg, her ankles are slightly swollen and urinalysis shows protein +. There is some tenderness under the right costal margin. Which one of the following is the most likely cause of her symptoms and examination findings?
