A 27-year-old woman undergoes a laparoscopic appendicectomy for perforated appendicitis. About 200ml of frank pus is found in the pelvis and washed out. The patient is given a five-day course of antibiotics (gentamicin, amoxycillin and metronidazole). Her postoperative recovery is delayed by a paralytic ileus. Seven days after the procedure she spikes a fever of 39.0°C and develops mucous diarrhoea persisting over the next three days. Which one of the following would be the most likely explanation of her current problem?
(A) Antibiotic-associated colitis
(B) Resolution of the paralytic ileus
(C) Staphylococcal gut infection
(D) Clostridium difficile infection
(E) Pelvic abscess
