A 30-year-old woman who has been homeless for the past 10 years presents to the Emergency Department complaining of fatigue, nausea, vomiting and abdominal colic. She has been feeling unwell for four months. She admits to currently living in a derelict old house. Examination reveals she has a motor neuropathy with wrist drop. Blood film shows basophilic stippling of the red blood cells. Which one of the following is the most likely diagnosis?
