A 32-year-old woman of 39 weeks gestation attends the antenatal day unit feeling very unwell with sudden onset of epigastric pain a/w nausea and vomiting. Temperature is 36.7C. On examination, RUQ tenderness is noted. On lab examination, mild anemia, low platelets, elevated LFT and hemolysis. What is the most likely diagnosis?
(A) Acute fatty liver of pregnancy
(B) Acute pyelonephritis
(C) Cholecystitis
(D) HELLP syndrome
(E) Acute hepatitis
