A 47-year-old Aborigine complains of blurring of vision. His blood pressure is normal, he smokes 40 cigarettes per day and drinks 100 grams of alcohol per day. There is no history of chest pain. On examination his blood pressure is 140/80 mmHg. The fundus is shown in the image. What is the most likely cause of this finding?
