A 60-year-old man is hospitalized because of progressive dyspnea during the past month. He has a 45-pack-year smoking history, but he has no other medical problems. On physical examination, he is cyanotic and has paradoxical respiratory movements of his rib cage and abdomen. Blood pressure is 140/78 mm Hg, pulse rate is 105/min, and respiration rate is 28/min. Jugular venous distention is present. The lungs are clear. The remainder of the examination is normal.
