A 60-year-old woman who has had rheumatoid arthritis for 35 years and has been managed on corticosteroids for the past 10 years, now complains of sudden onset of pain and swelling in the right knee which is warm and very tender. There is no other evidence of synovitis. What is the most likely diagnosis?
