A 67-year-old lady is admitted with a productive cough and confusion ten days after being in hospital with a stroke.

On examination her temperature is 39°C, BP 110/60 mmHg, pulse rate is 110/min, and respiratory rate is 22/min. She has a residual left sided weakness but no other neurological deficit. There is dullness on percussion at the right base with coarse crackles and an area of bronchial breathing. A chest x-ray revealed shadowing in the right lower lobe and a diagnosis of pneumonia was made.
