A 75 year old man with chronic bronchitis develops a bulge in his left groin. On examination, an elongated swelling is seen above the medial end of the inguinal ligament. When the patient coughs, the swelling enlarges, but does not descend into the scrotum. Which one of the following is the most likely diagnosis?
