A 78-year-old man presents to your clinic with hemoptysis for 2 weeks. His history is significant for smoking one and a half packs of cigarettes per day for the past 60 years. He denies having a fever or purulent sputum. He also reports a weight loss of 8kg over the past 2 months and increasing dyspnea. On physical examination BP is 165/90 mm Hg, and the rest of his vital signs are normal. The patient appears to have a full, round face with truncal obesity, and there is wasting and bruising of the extremities.

Cardiovascular and pulmonary examination results are normal.
