An 11-year-old boy complains of sudden onset of pain in the right side of the scrotum four hours ago, followed by nausea and vomiting. On examination, the right hemiscrotum is red and swollen and acutely tender, but no other abnormality is found. Which one of the following is the most appropriate next step in management?
(A) Treat him with a broadspectrum antibiotic
(B) Send urine for microscopy, culture and sensitivity
(C) Arrange an ultrasound of scrotum
(D) Arrange for a total and differential white cell count
(E) Arrange for surgical exploration of the scrotum
